How do I find my 401(k) or 403(b) plan disclosure?

Each provider posts their plan disclosure in a slightly different spot, but typically you can find it under a heading similar to “Plan Documents.” Some providers call the form an “annual disclosure document,” others a “participant fee disclosure” — or some variation of the two.

If you’re having trouble finding the document on your provider’s website, you can always give them a call and ask to be pointed in the right direction. Ask for the document that lists the investment options available to you in your plan and your plan’s fees.
